What this service is

Life Stage, Transition supports help you plan, prepare and move confidently through key changes—leaving school, starting work or study, moving house, shifting to more independent living, changing providers, or returning home after hospital.

Who it’s for

NDIS participants who want extra hands-on support to organise next steps, build routines, connect with services, and reduce stress for themselves and their families.

How we help (practical supports)

  • Transition planning & coordination – goal setting, timelines, booking appointments, linking you with the right services.

  • Skills for the next stage – routines, time management, money skills, cooking/cleaning basics, digital/phone skills.

  • Community & service linkages – GPs, therapists, training providers, Centrelink, housing, employment services.

  • Move & setup support – viewing properties, utilities, basic home setup, shopping lists and safety checks.

  • Work & study readiness – resumes, applications, interview practice, campus/workplace orientation.

  • Travel & confidence – route planning, public transport practice, safe travel routines.

  • Paperwork & advocacy – forms, evidence gathering, service agreements, attending meetings with you if requested.

What’s included

  • A simple Transition Plan with goals, steps and dates

  • A named key worker for consistent support

  • Regular check-ins and a short progress summary you can share with your coordinator or family
