Hospital Discharge Support – NDIS & Support at Home
Returning home after hospital can be challenging when the right supports are not yet in place. Glow Care Services For U can help arrange practical community-based support for eligible NDIS participants and older Australians receiving Support at Home services, subject to location, funding arrangements and current capacity.
Support after hospital discharge
Depending on assessed needs and service arrangements, support may include personal care, domestic assistance, transport assistance, meal preparation, medication assistance, community access, social support, respite and everyday living support.
Helping reduce service gaps
We work with participants, families, support coordinators, care partners, hospitals and other referral professionals to help plan a safe transition home. Where local capacity is available, we can discuss urgent starts, temporary coverage, replacement support and ongoing services.
NDIS hospital-to-home support
For NDIS participants, we can coordinate with the participant and their support network to understand approved supports, preferences, routines and goals before services begin.
Support at Home after hospital
For older Australians, we can work with Support at Home providers and care partners to support daily living at home in line with the person’s assessed needs and agreed service arrangement.
